| Scott M. Rocklage, Ph.D. - Chairman, 5AM Ventures | ||
| Dr. Rocklage has over 20 years of healthcare management experience with strategic leadership responsibilities that have resulted in the successful approval of three US NDAs (Cubicin, Omniscan, Teslascan) and six distinct NCE IND-level studies. Dr. Rocklage was formerly CEO of Cubist, President and CEO of Nycomed Salutar, President, CEO and Chairman of Nycomed Interventional, and held various R&D positions at Salutar and Catalytica. Dr. Rocklage currently serves as Chairman of Achaogen, Cubist, Ilypsa and Miikana Therapeutics. Dr. Rocklage is also a board member of the BIO Emerging Company Section and Massachusetts Biotechnology Council and is a member of the board of associates for the Whitehead Institute. Dr. Rocklage received his BS in chemistry from the University of California, Berkeley and his PhD in chemistry from the Massachusetts Institute of Technology. | ||

| Mr. Nussbacher has been an Affymetrix Fellow since 2000, having served as Executive Vice President of Affymetrix from 1995 and as Chief Financial Officer from 1995 to 1997. Previously, he was Executive Vice President for Business and Legal Affairs of Affymax Research Institute. In addition to Symyx, he is a director of Biospect Inc., Fluidigm Corporation, and Xenoport Inc. At Affymax and Affymetrix, he played a central role in the development and implementation of business strategies for creating value in technology-based companies. His experience includes commercial and scientific collaborations, intellectual property licensing, and public and private financings. Mr. Nussbacher holds a B.S. in physics from Cooper Union and a J.D. degree from Duke University. | ||

| Camille Samuels specializes in biotechnology investing at Versant. Camille joined Versant in early 2000, shortly after its formation. Prior to Versant, Camille was responsible for business development at Tularik, Inc. (subsequently acquired by Amgen) where she in-licensed two of the company's clinical-stage products and led Tularik’s Technology Acquisition Group. Before Tularik, Camille worked in business development/strategic planning at Genzyme and Millennium Predictive Medicine, and as a management consultant to healthcare and biotech companies at LEK Consulting. Camille currently serves as a board member of the following private companies: Achaogen, Kythera, Novacardia, as well as Novacea (NOVC), a public company. Camille was previously a board observer at several companies, including 5 AM, Alexza (public), Genomic Health (public), Parallele (acquired), and Syrrx (sold) as well as Pathwork Diagnostics (formerly Predicant Biosciences). Camille earned her Bachelor's degree in Biology from Duke University and an MBA from Harvard Business School, where she graduated as a Baker Scholar. | Bryan Roberts joined Venrock in 1997 and is now a General Partner in the Menlo Park, CA office focused on the firm's activities in healthcare. | Scott Minick is a Managing Director with ARCH, joining the firm as a Venture Partner in 1998. From 1995 until 1998, Mr. Minick was director, President and Chief Operating Officer of SEQUUS Pharmaceuticals, a fully integrated, public biopharmaceutical company. From 1994 to 1995, he served as a director, interim President, and CEO of OncoTherapeutics, a private biotechnology company. Prior to that, Mr. Minick co-founded and was a director, President, and CEO of LXR Biotechnology. Mr. Minick led LXR from startup through its IPO. From 1981 to 1993, he was an executive of Baxter Healthcare, holding various leadership positions in device, service, and diagnostics businesses, most recently as the President of the Latin America/Pacific Rim business unit of Baxter Diagnostics. Mr. Minick formerly served as Chairman of Xcyte Therapies and Executive Chairman of Amphora Discovery. Mr. Minick is a board director for FastTrack Systems, MedVantx, Anagen Therapeutics, and First Genetic Trust. | Mr. More joined Domain Associates as a Kauffman Fellow in 1996, and he became a Partner in 2000. Present board memberships include Esprit Pharma, Glaukos, Novalar Pharmaceuticals, NovaCardia, OmniSonics Medical Technologies, TargetRx and Windward Medical. Past board memberships include ESP Pharma (acquired by Protein Design Labs, Inc.), Proxima Therapeutics (acquired by Cytyc Corporation) and Onux Medical (acquired by C.R. Bard). Mr. More was also responsible for Domain’s investment in IntraLase (NASDAQ: ILSE). Additionally, he serves as a founding board member for Kauffman Fellows Program and serves as an Advisory Board member for Montagu Newhall Associates, Fresh Tracks Capital, Okapi Ventures and the Medical Industry Group, NVCA. From 1997-1998, Mr. More served as the Chief Operating Officer of Small Molecule Therapeutics, a Domain portfolio company subsequently purchased by Morphochem AG. From 1992 to 1995, he was with Pharmacia Biotech in sales. Prior to joining Pharmacia, Mr. More held a research position at Somatogen, a company developing a recombinant blood substitute that was subsequently bought by Baxter. While attending business school, he worked with MedVest, an early-stage venture group in Washington, D.C. Mr. More has a B.A. from Middlebury College, and received his M.B.A. from the Darden School of Business Administration at the University of Virginia. | ||
| J. Kevin Judice, Ph.D. - Chief Executive Officer, Achaogen | ||
| Dr. Judice was most recently head of the medicinal chemistry group at Genentech, where he provided strategic direction to the growing small molecule effort within the company. Prior to Genentech, he served as Vice President of Medicinal Chemistry at Theravance, Inc. (formerly Advanced Medicine, Inc.), a role in which he had line responsibility for more than 90 of the company's roughly 180 scientists. During his early years at Theravance, Dr. Judice served as project team leader for the group which discovered telavancin (TD-6424), a rapidly bactericidal agent which is currently in Phase 2 human clinical trials. Subsequent to that, Dr. Judice oversaw all of the medicinal chemistry efforts during a period in which two more compounds were advanced to clinical trials — including an asthma agent which was successful in Phase 2 clinical trials and a compound (currently in Phase 1) to treat overactive bladder disorders. An additional five compounds have been nominated as development candidates by Theravance since its founding in 1997. Dr. Judice received his bachelor's degree at Texas A&M, his Ph.D. at UCLA, and was awarded an NIH postdoctoral fellowship at Berkeley. |
